    Heat Intolerance and Temperature Sensitivity After Stroke: Why Hot Weather May Feel Different

    Does hot weather seem harder to tolerate since your stroke?

    Maybe you tire much faster outside. Maybe heat makes you feel weak, dizzy or drained. Maybe you sweat differently, or your affected side experiences temperature in a new way.

    We hear women in Stroke Sisters ask whether stroke can change the way the body responds to heat. The answer is more complicated than a simple yes or no.

    Stroke can affect temperature sensation, sweating and the autonomic nervous system. Extreme heat and dehydration also stress the cardiovascular system. But research does not establish “post-stroke heat intolerance” as something every survivor experiences.

    Let’s separate what we know from what we’re still learning.

    Can Stroke Affect Temperature Sensitivity?

    Yes. Stroke can change sensation, including the ability to perceive temperature. The National Clinical Guideline for Stroke recognizes temperature as one of the sensations that may be altered.

    Some survivors report reduced temperature sensation, unusual cold sensations, differences between affected and unaffected sides, altered sweating or increased sensitivity. Symptoms depend partly on where the stroke occurred and which pathways were affected.

    This does not mean every survivor becomes intolerant of hot weather, but it helps explain why temperature may genuinely feel different.

    Stroke Can Affect Sweating Too

    Sweating is controlled partly by the autonomic nervous system, which regulates functions we do not consciously control. Research on autonomic dysfunction after stroke documents changes in sweating, skin temperature and blood flow.

    If you notice a significant change in sweating or temperature sensation, mention it to your neurologist or rehabilitation team.

    Why Does Heat Make Me Feel So Exhausted?

    Heat makes everyone’s body work harder. You sweat, lose fluid and increase blood flow to the skin to release heat. For a survivor already managing fatigue, mobility challenges or another health condition, that added demand can make activity much more exhausting.

    It may be several things happening at once rather than a direct neurological “heat intolerance.” Hot weather may mean walking takes more energy, therapy feels harder, more breaks are needed and outdoor activities require longer recovery.

    Dehydration Can Sneak Up on You

    Hot weather increases fluid loss. Dehydration can cause headache, dizziness, weakness, fatigue and nausea—symptoms that can be especially unsettling after stroke.

    The CDC recommends staying hydrated rather than waiting until you already feel ill. If a physician has restricted your fluids because of a heart, kidney or other condition, follow that individual plan instead of general hydration advice.

    Some Medications Affect the Body’s Response to Heat

    Some medications used for blood pressure, heart disease and other conditions can affect hydration or the body’s response to heat. That does not mean you should stop taking them.

    If you are concerned that medication contributes to dizziness, dehydration or heat difficulty, speak with your healthcare provider or pharmacist. Never change or stop prescribed medication because it is hot outside unless your provider tells you to.

    Heat Can Make Exercise More Demanding

    A walk on a mild morning and the same walk on a 95-degree afternoon are not equivalent. Heat adds cardiovascular demands as the body tries to cool itself.

    • Choose cooler parts of the day
    • Consider shorter sessions and more breaks
    • Hydrate according to your medical guidance
    • Wear lightweight, breathable clothing
    • Move indoors when conditions are extreme

    Your indoor therapy exercises still count. So does walking in an air-conditioned building. There is no medal for exercising outside when it is dangerously hot.

    Practical Ways to Handle Hot Weather

    Plan Around the Weather

    Check the forecast. Morning or evening may be easier than the hottest part of the day. Pay attention to humidity too: high humidity makes sweat less effective at cooling the body.

    Rest Before You’re Depleted

    Do not assume your body will tolerate an activity the same way on a hot day. Stop and rest before you hit empty.

    Stay Cool and Hydrate Appropriately

    Use air conditioning, seek shade, wear breathable clothes and use a cool shower or damp cloth. Drink fluids regularly unless your healthcare provider has given you restrictions.

    Adjust Your Expectations

    Your body may have different limits now. Adapting is not weakness; it is listening to the body you are living in today.

    Heat Exhaustion vs. Stroke Symptoms

    Heat illness can cause weakness, headache, dizziness, nausea, heavy sweating and faintness. Stroke can also cause sudden dizziness, weakness and other neurological symptoms. Do not assume a new neurological symptom is “just the heat.”

    B – Balance: Sudden loss of balance or coordination

    E – Eyes: Sudden vision changes

    F – Face: Facial drooping

    A – Arms: Arm weakness or numbness

    S – Speech: Speech difficulty

    T – Time: Call 911 immediately, even if symptoms improve. Do not drive yourself.

    Know the Signs of Heat-Related Illness Too

    Heavy sweating, dizziness, headache, weakness, nausea and muscle cramping can signal heat illness. Heat stroke is the most serious form. Confusion, loss of consciousness or very high body temperature after heat exposure requires emergency care. Call 911.

    Does Hot Weather Increase Stroke Risk?

    Research has found population-level associations between extreme temperatures and cardiovascular events, including stroke. This does not mean walking outside on a hot day will cause another stroke. Risk depends on exposure, hydration, cardiovascular health, age, medications and other conditions.

    The practical takeaway is not to fear summer. It is to respect extreme heat. Pregnancy can also increase vulnerability to heat-related illness, so survivors who are pregnant or planning pregnancy should discuss precautions with their healthcare provider.

    When Should You Talk to Your Doctor?

    Mention major changes in sweating, one side feeling unusually hot or cold, difficulty sensing temperature, repeated heat-related dizziness or weakness, symptoms that interfere with rehabilitation, or concerns about medication in hot weather.

    A Final Thought

    If heat feels different after stroke, do not automatically dismiss the experience. Temperature perception and sweating can change, while extreme heat and dehydration add demands of their own.

    Move your walk to the morning. Take the break. Turn on the air conditioning. You do not have to push through miserable conditions to prove you are recovering. Sometimes self-care means knowing when it is simply too darn hot.

    Can a stroke make you more sensitive to heat?

    Stroke can alter temperature sensation, sweating and other autonomic functions in some survivors. Research does not establish heat intolerance as a universal post-stroke condition.

    Should stroke survivors drink more water in hot weather?

    Hydration is generally important, but people with heart, kidney or other conditions may have individualized fluid recommendations. Follow your healthcare provider’s guidance.

    Can stroke medications affect heat tolerance?

    Some cardiovascular medications can affect hydration, blood pressure or the body’s response to heat. Never stop medication because of hot weather without medical guidance.

    Can extreme heat increase stroke risk?

    Research associates extreme temperatures with increased cardiovascular and cerebrovascular risk at a population level. It does not mean ordinary heat exposure will cause a stroke in an individual, but extreme heat should be respected.

    How can I stay active when it’s too hot outside?

    Move activity indoors, exercise during cooler parts of the day, shorten outdoor sessions and take more breaks. Ask your rehabilitation team about safe alternatives.
